   General Information

Purpose

Our purpose is to assist your physician in restoring your health and preventing a recurrence of your injury.  This is best done in an atmosphere of congeniality, sincerity, and honesty.  We encourage you to ask questions about your treatment.  When we understand you and your goals, we can progress toward the common goal of improving your health.

Hours of Service

Monday thru Thursday 7:30 a.m. to 5:30 p.m.  Friday 7:30 a.m. to 1:00 p.m.

Practitioner Referral

Patients are referred by a physician, chiropractor, or podiatrist, and treatments are guided by that practitioner.  We will inform your referring practitioner of the treatment you are receiving.  A referral continues only for a certain amount of time and will expire after a specified number of treatments. Your referring practitioner can renew your referral.  Your physical therapist will discuss this with you.

Appointments

If you arrive early for an appointment, we will try to see you early.  If you are late for an appointment, your treatment may be shortened.  If you arrive without an appointment, we will give you the next available time.  We sincerely appreciate your cooperation in saving valuable time.

Appointments can be made during our service hours.  The primary reason for appointments is to make efficient use of everyone's time.

If you need to cancel an appointment, as a courtesy to other patients and the therapists, please call by 8:15 a.m. of the appointment day.

Conclusion of Treatment

We will notify your referring practitioner at the conclusion of our services by mailing a treatment summary. It contains a summary of the effect of treatment. If you want to know about your case summary, just ask your therapist.

Payment Policy

Rebound Orthopedic Physical Therapy is a preferred provider with numerous insurance companies.  If we are not listed as a provider for your plan, please call us to ensure the accuracy of the listing.  It is our policy to bill your insurance company and collect the portion not paid by the insurance company after each treatment.  Upon your first appointment, billing policy and payment plan will be discussed and formalized.  Remember that we are ready to discuss any payment problems or questions you may have, and work with you to make appropriate arrangements.

We also accept Workers Compensation and Automobile policies.

Rebound Orthopedic Physical Therapy supports both your health and financial needs.  We are happy to arrange payment plans for patients with limited or no insurance.  For payments, including co-payments, we accept:
